To get started, you may call us at 1-866-914-3700 to request an intake form and brochure, or you may print them out on your own computer and simply mail in the completed intake form.
Note: In order to view the PDF you must obtain a free copy of Adobe Acrobat Reader. Many computers come with Acrobat Reader pre-installed.
The intake form should be completed and mailed to the following address:

The brochure explains the different programs offering FREE preschool so you can decide which program you are interested in. We will forward the completed form to the program of your choice.
What to Expect When You Call 1-866-914-3700 (our toll free number):
You will reach a recorded message that asks you to leave your name, address, phone number.
Within 1 business day, we will mail you an Intake Form, a Brochure identifying all of the preschool programs in the Partnership, and a Letter explaining the free preschool program.
When we receive your completed intake form, we will process it and forward it to the first available program you chose. Please be sure to clearly identify your 1st and 2nd choice on your intake form.
The first available program you chose will contact you to complete the application process when they receive the intake.
